For this activity, YOU are the respiratory therapist on shift at your hospital, you are assigned the patients listed below. Each patient must be INITIALLY ASSESSED, treated using the CARE PLAN YOU CREATE, and monitored for TOLERANCE TO TREATMENT/THERAPIES you recommend. POST ASSESSMENT must be performed to determine if your recommendations are EFFECTIVE FOR THE PATIENT and EDUCATION must be given to ensure the patient continues to improve and maintains a well state or condition AFTER DISCHARGE.

Initial Assessment – document the signs and symptoms observed or reported
Diagnostic Testing and Results – what tools, equipment and lab tests did you use to diagnose or rule out disease or system/organ dysfunction for THIS patient? Each test will include the results you obtained from each test
Treatment Care Plan – create a care plan and select the treatments you recommend for every abnormal finding
Monitoring Therapeutic Effectiveness and Tolerance – document the assessment method, tools, equipment and lab tests you recommend for assessing the effectiveness and tolerance of your care plan
Education and Self-Management of Disease and Symptoms, and Reduced Risk for Hospitalization – describe how you will ensure the patient continues to improve and remains well after leaving your care
Please be detailed in your selections of tests and treatment options, document the results for each test prescribed. Include dose and frequency of any drugs recommended and document the therapeutic outcomes, how you determine the condition is improving or worsening with your prescription.

Patient Assessments – External Assessment:
Level of Consciousness, PERRLA, facial droop, distended neck veins; barrel, excavatum or carinatum chest, scoliosis; pale, blue, or reddish skin or turgor; digital clubbing, cough, pedal or general edema; etc.
Heart Rate – Pulsus paradoxus?
Respiratory Rate
Respirations – Pursed Lip breathing, accessory muscle use, shallow, abnormal patter (Kussmaul’s, Cheyne-Stokes, apnea time, etc.)
Breath Sounds, resonance
Pulse oximetry
Blood Pressure
Temperature
EKG
Diagnostic Testing and Results:
Imaging: Chest x-ay, ultrasound or doppler, CT scan, MRI
Laboratory tests and Tissue sampling: blood, urine, sputum sampling and correct culturing tecniques for each, skin or allergy tests. ABG, VBG, Capillary gas results, CBC, WBC, glucose, Electrolytes and Enzymes
Pulmonary Function Tests – bedside and in Lab – PEFR, MIP, exhaled volumes, capacities, diffusion
Heart Catheterization, cardio or Pulmonary stress testing

Medications
RT modalities (inhaled meds; hyperinflation, bronchial hygiene)
Oxygen or assisted ventilation
IV or P.O. meds
Monitor/Assess Improvement:
Repeat testing or monitoring of signs/symptoms (oxygenation, PEFR, MIP, VC, cough results, BS, etc.)
Material and content to reduce hospital length-of-stay; improve adherence to medication regimen and quality of life; reduce re-hospitalization; (smoking cessation, MDI/DPI use, hyperinflation and bronchial hygiene devices, monitoring symptoms with equipment/devices.
Education:
Asthma usually managed, now with exacerbation of symptoms in emergency dept. ER ROOM 7
Suspected cystic fibrosis in a 3-year-old male
Suspected Guillain Barre in 32-year-old female
32-week gestation newborn by C-section
Suspected tuberculosis of lung
Suspected pulmonary embolism in 26 yr old female with 20 yr pack history of smoking tobacco
